Just thinking it might be worth looking at the ReactOS implementation of NTVDM for Windows usage and 16-bit doors. Should be possible to simply include their ntvdm.exe and use that for Windows targets as opposed to relying on a third party Windows source leak for the implementation. The ReactOS does 486sx emulation (no floating point), but would likely cover (most) door scenarios and could be a good default option for Windows (64bit in particular) users.
